Liability and Medical Waiver & Photography Release

I hereby give consent for my child to participate in Wizards Sports Club Summer Beach Tournament at Point Pleasant, NJ. I assume all risks with regard to participate in the session(s) and/or games in which my child will participate. I release, indemnify and agree to hold harmless Wizards Sports Club, Inc., its directors, coaches and trainers, and the Borough of Point Pleasant from any liability that may result from participation in this event. I attest to the following: That the above information is correct and in the event of a medical emergency, I authorize Wizards Sports Club, Inc. to seek emergency medical care for my child as deemed necessary. I understand that Wizards Sports Club, Inc. does not provide accident insurance. I do hereby authorize Wizards Sports Club, Inc, and its assigns to utilize any and all photographs, pictures, videos or other likeness of me or anyone assigned guardianship to me, as they deem appropriate in its promotional materials or team films.
